Pre-Internship Guide (PDF)

The purpose of this guidebook is to provide important and useful information for pre-interns in the Secondary Teacher Preparation Program. The guidebook gives an overview of the program and policies that apply to the pre-intern teacher candidates.

Required Secondary Teacher Program Courses (PDF)

Over the course of the five year teacher certification program you take a series of professional education courses while you complete the university undergraduate degree in your subject matter major.  Two of the courses (TE 150 and TE 250) are completed prior to being admitted to the teacher preparation program.  After TE 150, each of these courses includes both on-campus instruction and site-based field work.

Honors Option for TE Coursework Form (PDF)

Use this form as the project agreement form if you opt to take a class for honors.

Background Check Form (PDF)

You may not begin your field placement until a criminal background check has been completed. Please print a hard copy of the form, completely fill it out, and return this form to the Secondary Teacher Preparation Program Secretary in 347 Erickson.

Conviction Disclosure (PDF)

Students, interns, and teacher certification candidates are obligated to report convictions to the Certification Officer in the Student Affairs Office. You must submit a Conviction Disclosure Form prior to (1) admission to the teacher education program and (2) internship placement. Please click on the link above for more information.

Welcome Guide (PDF)

This guide provides internship information for both prospective interns and mentors for use in the spring and summer preceding the internship.  It includes some details about enrolling for the internship coursework and about the fingerprinting requirements. It also includes planning checklists for the initial meeting between intern and mentor and other checklists and resources to ensure a successful start to the internship.

An internship includes many key participants who take on challenging relationships that become the basis for successful professional learning experiences for those involved. This guide is designed to provide information, a structure, and guidance for internship participants - interns, mentor teachers, field and course instructors - to help them get the most out of this joint venture.

Exit Performance Description (EPD) Coversheet

The final evaluation of the intern's performance is formally written as an Exit Performance Description (EPD). Both the field instructor and the mentor teacher are responsible for producing individual EPDs representing the intern’s performance for the year based on the Program Standards. An EPD is not a letter of recommendation. It is an evaluative document designed to assess an intern's strengths and weaknesses as a prospective teacher.  The EPD is the documentation that supports the final grade in TE 502. The intern may choose to include this document in his/her portfolio or credential materials. Please contact the Secondary Team at team4@msu.edu if you need access to this form.

Intern Assessment of Progress Form

This form is used as a tool for discussion. This form serves as a way to prepare and organize thoughts related to an intern's growth and progress at the mid-term and final conferences each semester. Please contact your field instructor for more information.

Field Instructor Progress Report Form (PDF) (RTF)

The Field Instructor Progress Report Form is used to report progress of intern as observed in the field. This form is then emailed by the field instructor to the program coordinator and Secondary Teacher Preparation Program Secretary.

Travel Log Form (XLS)

Mileage claim reimbursements will now be processed using the simplified Travel Log form below (Excel format) which automatically calculates mileage entered at the current rate of 56 cents per mile (effective January 1, 2013). This allowance is intended to cover the prorated cost of the vehicle, fuel, insurance and vehicle maintenance. You can access the Manual via the Controller's Office home page at: www.ctlr.msu.edu
